Applicants must submit the following materials no later than the extended deadline of May 27th, 2011:

A) At least one recommendation form from teachers and/or local community members.

B) Complete the student application form with signed parental approval and contact information, including email if available.

C) OPTIONAL: ONE of the following that answers the question:

Why should youth be considered assets in times of emergency?

Paragraph– minimum 200 words in length
• Clearly addressing the question.
• Typed or neatly written in ink.
• Spelling, grammar, punctuation, and rules of good composition apply.
Poster – No larger than 11” x 17”
• Clearly addressing the question.
• Good use of imagination, color, and design.
Video – 2-4 minutes in length
• Clearly addressing the question.
• Imaginative, interesting, creative.
• Good application of video production and editing principles.
